Every serious injury sits on a base of hundreds of near-misses.

Decades of safety research keep finding the same shape. For every serious injury there are more minor ones, more property-damage events, and hundreds of near-misses and non-conformities underneath. Qonyx reads that base continuously, so a rising pattern is caught while it is still a near-miss.

This is what "learns from your operation" looks like in practice. The checklists, photos, and voice notes a crew already captures fill the base of the pyramid: the small non-conformities that rarely reach a report. Qonyx watches that layer as it builds.

The domino theory of accident causation framed it early. An incident is a chain of events, and every non-conformity removed is a domino that never falls.

The read stays at the task. When the same task starts logging repeat non-conformities, like work-at-height across one week, Qonyx flags the rising exposure and recommends targeted training. It measures the operation, not the person, and the decision to act stays with the supervisor.
